In order to enhance our service to you we are changing our online services. Please register for the new service by downloading this form, completing it and bringing it to the surgery together with ID.
For patients already registered for this service, the existing Vision Online Services website is being replaced with a new website called Patient Services in the near future.
Once the new website becomes operational you will be automatically transferred to it if you try to log in to the 'myvisiononline' website.

Please allow three full working days between requesting and collecting your medication.
Prescriptions which are not on your repeat list may need to be authorised by your own GP and may take longer than 3 days.
You can request your repeat medication by indicating the items you require on the tear-off slip of your prescription, and posting it to us or handing it in at the practice reception. If you enclose a stamped self addressed envelope we will post your prescription to you.
You can also ask your local pharmacy to request and collect your prescriptions for you. Telephone your local pharmacy for more information.

Patients on repeat medication will be asked to see either a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications.
