NEW

In order to enhance our service to you we are changing our online services. Please register for the new service by downloading this form, completing it and bringing it to the surgery together with ID.

For patients already registered for this service, the existing Vision Online Services website is being replaced with a new website called Patient Services in the near future.

www.patient-services.co.uk

Once the new website becomes operational you will be automatically transferred to it if you try to log in to the 'myvisiononline' website.

Alternatively you may email your request directly to one of the following email addresses

Portland.scripts@nhs.net

hatherley.scripts@nhs.net

You may still use the current systems for the time being, however these will be phased out in the next few months.

Please allow three full working days between requesting and collecting your medication.

Prescriptions which are not on your repeat list may need to be authorised by your own GP and may take longer than 3 days.
