New Patient Registration
New patients living within our area will be eligible to join our practice.
When you come to register you must bring with you two forms of ID, one with a photograph and one which must show your current address. Your address and postcode must be within our practice boundary. This is important for both you and the practice, so that you may access the full range of NHS care and services.
Please note that we will also require you to complete a health questionnaire and some other forms when you come to the surgery and before we register you with the practice.
If you move outside our practice area, you will be asked to re-register with a GP local to your new address. Existing patients who live in Cheltenham but outside our new boundary will be asked to register with another GP surgery. Please refer to our postcode checker on the 'practice area' tab.
