General Information
Welcome
to
The Portland Surgery is our main surgery and usually the on-call Doctor will be based there. We also run a busy branch surgery in Up Hatherley.
As
all doctors do surgeries at both
sites, your registered doctor may not always be available at your usual surgery.
However, a message can be left for your doctor to contact you by telephone if
necessary.
This page contains information on the following. Click on the link to jump to that section.
Complaints Procedure Disabled Access Practice Charter Private Medical Reports Repeat Prescriptions Sick Certification Video Recording Consultations
Occasionally consultations will be recorded as part of General Practitioner training. No recording of consultations will take place without a patient's consent and intimate physical examinations will not be recorded. The consultations will be used for training purposes only, will be turned off on request, and deleted after a consultation if a patient requests.
If you are on regular medication you can request your new supply if one of three ways:
1. by email
2. by letter.
3. by calling at the surgery.
4. by asking your Pharmacy to order it for you
Please try to give at least 48 hours' notice.
The surgery is open Monday to Friday. Repeat prescriptions cannot be ordered or collected on Saturday mornings.
If you have to pay for your prescriptions you may find it cheaper to purchase a prepayment certificate.
St Paul's is a custom-built building with easy access and lifts to the first floor (where the Portland Practice is located). Hatherley enjoys similar access facilities.
If your disability means that you are unable to come to the surgery, we are happy to visit you at home.
The practice has an accepted complaints procedure and should you have a complaint please write to Laurella Parffrey, Practice Manager at Portland Practice. Your complaint will then be dealt with quickly and efficiently.
Our charter is on display at both surgeries. Our aim in the practice is to give you the best possible service and care. To do this we need to work together. Feel free to read our charter on your next visit.
An employee can self-certify for short term illness (up to six working days). After this a Doctor's Medical Certificate is required. Any request from employee or employer for a doctor's certificate earlier than six days will be by private certificate only, for which there will be a small charge.
Medical Reports and Non-NHS Examinations
Medical Reports (insurance companies, mortgage companies etc) are frequently undertaken by GPs as they have access to your medical history. They will not be done without your written consent.
These reports are, however, outside the doctor's contracted medical duties and are consequently done in their spare time. Although the GPs will try to do them as soon as possible, priority has to be given to their contracted medical services. If the Doctor is happy to do them, a fee will be charged as recommended by the British Medical Association.
